form DS3025 NEED ANSWERS

we are filing for adjustment of status for k1 visa , when i did my interview doctor give ,me sealed envolope and one form document vaccination worksheet it is ds 3025. after i travel i give the yellow packet in poe , now we wanna file adjustment of status do i can include copie of ds 3025 in package < same in foto bellow

do all petition goes to chicago office ,thank you

Send in a copy of the DS3025 with the packet. since you had the medical within a year, and you came on a K1, there is no reason to get another medical examination.
