Another Vaccination Supplement Question

My wife has an AOS interview coming up, and one of the things it asks for is the I-693 and vaccination supplement in a sealed envelope (unless already submitted).

She came on a K-1 visa, and they gave her the original vaccination supplement, but it's not sealed in any way.

What do we need to fulfill the requirements here?

Also, is the Affadavit of Support I-864 required again? We sent it in with the original application.

The Vaccination Supplement issue is probably the most confusing one in the whole AOS process.

I had a copy of my Vaccination "Worksheet" from my K-1 medical exam, and submitted that with my AOS application and got approved with no problem.

For my K-2 daughter's AOS interview, I took her Vaccination "Supplement" to her interview, which I had prepared by a civil surgeon here in the US. I had submitted her Vaccination "Worksheet" from her K-2 medical exam with her AOS application, but then received a letter from USCIS saying that I needed the sealed medical and Supplement, so I got the Supplement done just to be safe.

They did not ask me for the Supplement at her interview..... I voluntarily gave it to them because I figured it was no good to me and I had paid $35.00 to get it, so I thought they may as well have it.

I have heard a lot of people say they had the Supplement and were never asked for it, and others were asked for it and didn't have it. So.... it's up to you whether you want to get one done or not.

As for the I-864, you do not need to take another one if you already submitted one with your original AOS application.

Here's some USCIS info on the Vaccination Supplement (The Worksheet is what you get at the over seas medical)

FAQ's And here is what it says about K1 applicants:

Ks: If you were admitted to the United States as a fiance(e) (K-1), child of a fiance(e) (K-2), Spouse of a U.S. citizen (K-3), or child of K-3 (K-4), and received a medical examination prior to admission, then you do not require another medical examination as long as your application for adjustment of status (Form I-485) is filed within one year of your overseas medical examination. You will, however, be required to submit a vaccination supplement with our adjustment of status application. The vaccination supplement must be completed by a designated civil surgeon.

Some people have been asked to submit it, some have done just fine with the Worksheet. It really depends on the Office and the person interviewing you.

We have the vaccination record that was completed as part of the K-1, but it's not sealed in any way. We made a photocopy of it with the AOS application.

We never completed the vaccination worksheet as part of the application - just submitted the photocopy of what was given to my wife.

Should we just bright the unsealed vaccination record with us? My main concern is that it says "sealed". How would we go about getting it resealed?

The Vaccination Worksheet you have is only a copy of the real one, which was in the brown envelope you handed over at your POE. So, don't seal it, as the Officer at the interview would be wondering what the heck was going on!

Basically it's up to you, do you want to spend the time and money to get the Vaccination Worksheet info transferred to the Vaccination Supplemental by a designated U.S. civil surgeon? The only thing that'll happen if you don't, is that the interviewing officer will hold off approving your AOS until you submit the Vaccination Supplemental.

I got my Vaccination Worksheet info transferred to the Vaccination Supplemental, simply because I felt more secure having this done for the interview.

* The Vaccination Worksheet is what you get from your overseas medical

* This can later be transferred by a designated civil surgeon here in the U.S to what is called the Vaccination Supplemental.

The Vaccination worksheet you got during the K-1 proccess is NOT the suppliment they are refering to!

The suppliment is done by a designated US civil surgeon, and they take the vaccanation information from the worksheet you have now, and TRANSFER it to the Supplimental form. It costs about $20-50 depending on where you go.
