Ok, I know this topic has been here for many many times, but I still have questions.. I know one of the forums said that we don't need to send the medical for AOS, but another one still says that we need to fill the I-693.

I was able to get a copy of DS-3025 form from the exams I had to have for K1 visa. I sent it to a civil surgeon here, because I'll go there to have the I-693 filled.

The nurse said that some of the vaccines are missing, but it's for example an influenza vaccine. My question is - is it really necessary? It's voluntarily in the Czech Republic and I know lots of people who had big problems after this vaccination - they were sick for a long time. I really don't want to go through it.

Another ones, the nurse said I have to have are these: (I'm sure I have tetanus and MMR vaccine, she probably just didn't see it in the records)

- Tetanus/diphtheria or Tdap

- A 2nd MMR vaccine
- Influenza vaccine
- RPR/VDRL bloodwork
- TB skin test or Tspot (to check for tuberculosis)
I guess they won't fill the I-693 for me if I don't do it, right???
So I don't know what to do. I'm ok with the bloodwork, but not the vaccine.
(I know that I have all vaccines ok, because anyhow I wouldn't get K1 visa, so I don't know why I have to have another ones, or different ones now... )
Thanks!!

This answer is not correct for someone adjusting from a K-1. As long as your medical abroad was within a year of filing for AOS, you do not need a new medical or any parts of one, such as blood work or another TB test.

Hi! Advice! Ask your doctor to check for tuberculosis through your blood! They have to draw blood anyway, and it saves time , you wont have to do tests! Doctor suggested it for me because he knew that I would get the skin test spot wet haha, I'm clumsy. And if that happened he would have sent me to xrays.

p.s. hope you're prepared, the 1st two vaccines hurt like someone punched you in the shoulder! :(

Hi Maruska,

You do not need full I-693, but you will need the vaccination record as per I-485 instructions say - read below in red to see which vaccinations are required.

As noted in discussions on VJ people sometime get RFE for submitting only vaccination record without I-693 completed so it is more a question of personal preference whether you choose to go beyond the requirements and do the whole I-693 or only what is officially required (just the vaccination supplement).

If you only want to do the vaccination supplement and the doctor says they will have to do the whole I-693 then find another civil surgeon who will agree to fill out just the vaccinations supplement and not charge you for the entire exam :)

I-485 Instructions, page 4, 7C:

https://www.uscis.gov/sites/default/files/files/form/i-485instr.pdf

"If you are a K-1 fiancé(e) or K-2 dependent who had a medical examination within the past year as required for the nonimmigrant fiancé(e) visa, you only need to submit a vaccination supplement, not the entire medical report. You may include the vaccination supplement with your Form I-485. "

I-693 Instructions, page 9-10, specifies further:

Part 5. Vaccination Requirements

All applicants for adjustment of status must present documents showing they were vaccinated against a broad range of vaccine-preventable diseases. The civil surgeon will review your vaccination history with you to determine whether you have had all the required vaccinations. Make sure you take your vaccination records with you to your appointment with the civil surgeon.

NOTE: Do not attempt to meet the requirements before the civil surgeon evaluates you, in case any of the required vaccines are not medically appropriate for you.

You can find a list of the required vaccines at www.cdc.gov/immigrantrefugeehealth/exams/medical-examination- faqs.html.#6.

If you never received certain vaccines, or you are unable to prove you received them, the civil surgeon can give them to you. You also have the option to ask your family doctor to administer those vaccines to you after your evaluation by the civil surgeon. If you choose that option, show the records to the civil surgeon to note on Form I-693.

If you initially did not have documents proving you received all the required vaccines, but later submit those documents, USCIS may grant you a waiver based on the civil surgeon's certification on Part 7. Vaccination Record of Form I-693. USCIS may also grant you a waiver if the civil surgeon certifies that it is not medically appropriate for you to have one or more of the required vaccines.

HHS has determined that a vaccine is "not medically appropriate" if :

  1. The vaccine is not recommended for your specific age group;

  2. There is a medical reason why it would not be safe to have the vaccine (for example, you are allergic to eggs and/or yeast or you had bad reactions to prior vaccines);

Form I-693 Instructions 03/30/15 N Page 9 of 11

  1. You are unable to complete the entire series of a required vaccine within a reasonable amount of time; or

  2. For the influenza vaccine, it is not the flu season.

If you object to required vaccinations because of sincerely held religious beliefs or moral convictions, you may apply for a waiver of these requirements. If you hold these objections, inform the civil surgeon that you will apply for a waiver. If USCIS denies the waiver application, we may also deny the immigration benefit that you are seeking. For more information about these waivers, visit the USCIS Web site at www.uscis.gov.

AND Finally below links list out which vaccines you need:

USCIS specified vaccination requirements here:

https://www.uscis.gov/news/questions-and-answers/vaccination-requirements

Under the immigration laws of the United States, a foreign national who applies for an immigrant visa abroad, or who seeks to adjust status to a permanent resident while in the United States, is required to receive vaccinations to prevent the following diseases:

  • Mumps
  • Measles
  • Rubella
  • Polio
  • Tetanus and Diphtheria Toxoids
  • Pertussis
  • Haemophilus influenzae type B
  • Hepatitis B
  • Any other vaccine-preventable diseases recommended by the Advisory Committee for Immunization Practices

CDC further specifies requirements under their FAQ section:

http://www.cdc.gov/immigrantrefugeehealth/exams/medical-examination-faqs.html#6

"What vaccines/boosters are required for immigrants and those applying for adjustment of status in the United States?

As part of the medical examination for immigration, all immigrants are required to have an assessment for the following vaccine-preventable diseases: mumps, measles, rubella, polio, tetanus and diphtheria toxoids, pertussis, Haemophilus influenzae type B, rotavirus, hepatitis A, hepatitis B, meningococcal disease, varicella, influenza, and pneumococcal pneumonia. Persons already in the United States applying for adjustment of status for permanent residency, including refugees, are also required to be assessed for these vaccine-preventable diseases.

Do all vaccine series have to be completed before vaccination requirements are fulfilled?

For vaccines requiring a series, only a single dose is required for immigration purposes. However, the Civil Surgeon should check the “insufficient time interval” box on the I-693 Form, and also advise the applicant to complete the series in the future.

Oh, wow!! Thank you so much for the detailed description!

I believe that we have all the required vaccines from the Czech Republic - at least all of them from the list (just not sure about the influenza one) - and I guess I can do the bloodwork again.. I think they just want to make sure, but I'll try to call another civil surgeon and see what they have to say.

This is the only thing that is missing to be able to send AOS, so I just want to have it done and move on.

Thank you one more time! Good luck with yours if you're also in the applying/pending phase! :-)
