Timeline

My fiancee did her medical exam but never received any vaccination due to shortage. I am looking to file AOS which require vaccination record. She does not have anything kind of form. All she got when she entered the US is a stamp on her passport next to her visa and a hand written Alien number on her passport.

From what I understand she doesn't need another medical exam (I-693) since she recently did her within a year.

She still need her vaccination. Do I just print out DS-3025 form and take it to a civil surgeon to get all her vaccine?

This is what my gal did, others opinions/experience may differ. I had her go to the local health department and got all her needed vaccination/immunization shots and these shots are age-based or based on her age. You gal will probably get the MMR shot(s) and maybe up to 2 other shots for a total of 3 and should be around $100+/-. Best of luck!

You will need to bring to your civil surgeon form I-693, not DS-3025. The civil surgeon will give her the required vaccination(s), sign, and seal the I-693 form in an envelope. You can then turn it in with your AOS paperwork.
