Can't help with question 13 because we are not in that situation.

But, affidavits aren't really that important and don't add much value to your petition. Photos of the two of you together over the course of time (not 20 photos taken on the same day), ongoing communication and proof of travel to see each other are all worth a lot more than affidavits.

Timeline in brief:

Married: September 27, 2014

I-130 filed: February 5, 2016

NOA1: February 8, 2016 Nebraska

NOA2: July 21, 2016

Interview: December 6, 2016 London

POE: December 19, 2016 Las Vegas

N-400 filed: September 30, 2019

Interview: March 22, 2021 Seattle

Oath: March 22, 2021 COVID-style same-day oath

Now a US citizen!

Even if your wife got her USA passport through her father, she must still have a certificate number. When citizenship is issued there should be some sort of paperwork to show it. You can request a Replacement Naturalization/Citizenship Document. Use the N-565 application to request this. Here is the link: https://www.uscis.gov/n-565

No, this is not true. The OP's wife became a citizen automatically when her parent became a citizen and she was a permanent resident under 18. There was no "issuing" of citizenship. There was no process or paperwork.
