Ghana Passport Change of Name

Timeline

My wife is trying to change her name in her passport to match the name on her Green Card.

I sent the forms yesterday and they received it today but I later realized that I did not notarize the "witness" part of the form

My questions are.

Do I have to notarize the form and the pictures?

Can I quickly refill another application form, notarize it and send via fedex to them ( they'll get it first thing monday morning) so they'll replace it with the un-notarized form?

That way they won't have to return the entire application back to me.

I'm referring to question number 15 on the passport application form.
