Hey again, everyone.

As Im waiting to get visa from Algerian Consulate in New York, I am reading up on US Embassy in Algiers, Algeria.
In my packet 3 e mail I got medical exam instructions but no forms. As i'm reading instructions, i noticed on second page in French (came in english and french) but French page had a section in the end with forms and form numbers.

Form DS-2053 (Medical Examination for Immigrant or Refugee Applicant,) Form DS 3024, Form DS 3025, Form DS 3026. Again this is noted in section in French on Instructions page.

Reading the english instructions,, it says, "Applicant should attach these instructions to a blank medical certificate and envelope addressed to the Embassy and should give documents to doctor when applicant goes in for an appointment"

What "blank medical certificate" are they talking about? there were no forms attached to e mail. Just 1.Instruction letter

2.Doctor contact information and medical exam instructions

Anyone know?

Do u mean visa from algeria? Did u have your interview yet? I know u must get a medical exam before interview. You take your interview letter. There are 2 doctors listed they accept. If you had a timeline done it would really help.

1/18/2016 paid aos fee

1/28/2016 review 261 to get iv fee unlocked ( unlocked once I did review,was taking to long)

2/1/ 2016 sent AOS & IV packets together to nvc while I waited for iv fee to open to save time.

2/4/2016 paid IV Fee and waited to do ds260

2/3/2016 nvc received aos and iv packets(scan date)

2/7/2016 ds260 completed and now waiting for cc

♧CASE COMPLETE. .. march 8th

3/11/15 interview email

4/20/15 shots 4/25/15 rest of medical exam

INTERVIEW APRIL 27TH @8:30

April 27th 221g for additional documentation and placed in administrative processsing.

June 20th submitted evidence requested

June 23rd case was touched by embassy

August 1st case was touched again.

AUGUST 16TH embassy called and wants a new co sponser yet he met requirements.

Found a new co-sponsor

Handed in aos sept 11th

Set 18th another update

Sept 28th update and embassy called

SEPT 29TH ISSUED FINALLY

OCT 2nd visa in hand

Sorry. I'll do my timeline. I'm a US citizen just waiting for a tourist visa to be issued so I can meet my husband in algeria. He's the immigrant. K3 visa. In the instructions it says to give these empty medical forms to physician for exam but I can't find the forms. Where would I get them? I know to see the physician u need to make an appointment for interview which I haven't done for my husband yet because he will be arriving in algeria on thursday, God willing.

But the applicant needing to give empty medical forms to physician ...I don't understand

I never had to give medical forms for a tourist visa for algeria. I have gotten 2 for tourist visa.

Let me try to start over, I'm applying for my husband to come into usa. His k3 case is in Algeria at the us embassy. In the instructions attached to e mail from us embassy in Algiers it says for applicant (my husband) to give blank medical forms to physician to fill out.

Which forms? They weren't attached to e mail

After crazy researching , finally got it.

Just in case anyone else has this problem, come to find out Department of State changed form numbers on the 2 that I would search for and would come up but were expired.

Hopefully, this info will help someone in the future....

for medical, you'll need the following forms:

DS 3030 (xray and tb) replaces old DS 3024 Form

DS 2054 (Medical exam) replaces old DS 2053 Form

DS 3026 (Medical History and Physical Exam)

DS 3025 Vaccinations Form

Forgot to add: The new (revised) forms were mentioned on Department of State (DS 2054) website as well as CDC website (mentions all revised forms to the right when you click link below)
