I-485 Request for Initial Evidence (RIE)

My wife is a Chinese citizen. We submitted all documents for an I-485 so she can obtain her green card.

We just received an RIE that states:

"You have submitted a Family Census Register (FCR), Family Relation Register (FRR), or a Notarial Certificate. However, the evidence you have submitted does not contain the applicant's name. You must submit a Family Census Register (FCR), Family Relation Register (FRR), or a Notarial Certificate that contains the applicant's name."

We do not know what this means because the affidavit that we submitted DOES have here name! My wife does not have a Chinese birth certificate (she was born before birth certificates were issued in China), so we submitted an affidavit from her parents stating when and where she was born. I can think of two things that this RIE may be asking for:

1) they are asking for additional evidence regarding her birth, such as an official government letter

2) On her passport her first name is mis-spelled with a space between the first 3 letters and the last 3 letters - like Geo rge (obviously not her name). On her affidavit it is spelled George. Could this be the problem?
