Hi everyone,

I'm sure this forum sees a billion of these topics, but I just need the reassurance that my application is ready to go!

How does this table of contents look?

Packet for Form I-485, Application to Register Permanent Residence or Adjustment of Status includes:

  1. Cover Letter
  2. Payment (personal check) - $1,070
  3. Form I-485
  4. Copy of passport
  5. Copy of NOA2 "Approval" for I-129F
  6. Electronic I-94
  7. Copy of birth certificate
  8. Copy of marriage certificate
  9. G-325A with two passport-type photos
  10. Form I-693, Medical Examination of Aliens Seeking Adjustment of Status
  11. Form I-864, Affidavit of Support
    1. With proof of US citizenship
    2. Tax return for most recent tax year
    3. 3 recent pay stubs
Packet for Form I-765, Application for Employment Authorization includes:
  1. Form I-765
  2. Copy of passport
  3. Two passport-type photos
Packet for Form I-131, Application for Travel Document includes:
  1. Form I-131
  2. Copy of passport

Do I need to include my DS-3025?

If you got an I-693, then the DS-3025 is superseded by it. No need to include old news. People include the visa vaccination form (DS-3025) when it shows they already had the required shots and aren't going to see a Civil Surgeon to do an I-693.

Can I make one tiny comment on your well organized list?

You really don't do a "table of contents" as such with these submissions. A cover letter does nicely if you want to list what you have included in the envelope. So it would be more appropriate to use your list as the cover letter with this modification

4. etc

I would take "cover letter" out of the I-485 pack because a cover letter "covers" the entire contents of a mailed submission. By definition, it goes on top. Clip your check to it. It is redundant to make a table of contents to show you have a cover letter.
