I'm so confused with this requirement, form I-693. I have the copy of my x-ray in a cd and my vaccination record worksheet from the Philippines when I had my medical examination in St. Lukes Medical Hospital. Should I still look for a civil surgeon to sign and certify the form I-693? On my vaccination record I have the name and signature of the panel physician. Please help me. I need to file my adjustment of status on Monday. My legal status here as k1-visa holder will expire on February 19. Please help me. Thanks!

First of all, you do NOT need to file for AOS on Monday if your k1 status expires. Your K1 actually expired the day you entered the US, and your I-94 is what has the 90 day deadline--that deadline is in place for you to get married, and it has no bearing on your AOS application. There is no time you must apply to adjust status, so I would wait until you have all the necessary documents and then send your application in. Technically, you are out-of-status, but the possibility of you being deported is very unlikely.

Secondly, there is a thread in this forum that covers whether or not you need to complete form I693. There are certain vaccinations you need for your AOS application that you did not need for the K1 visa. You do NOT have to complete another medical, as the K1 medical will suffice. You won't need your X-ray for AOS--keep that for your own records. I would take a look at the copy of your DS3025 vaccination records and compare it to the requirements for AOS based on the thread in this forum and the CDC requirements. Many applicants who have all the vaccinations for AOS from their ds3025 do not need to be examined by a panel physician and can just supply a copy of this sheet in their AOS application. Uh, as long as you married within the 90 day period after entering the USA you will not have any problems filing the AOS. And, depending on what you read your I-693 may or may not be needed to be updated. To be safe, I would do the I-693 thing, see the USCIS CS and get his/her stamp of approval. And, do all of this when the "medical" taken at St. Lukes is still valid - if possible!

Just submit a copy of your DS-3025 with your AOS application. You do not need to send in an I-693.
