Mistake on G-325A example form?

Timeline

As you know, the G-325A wants you to list NAME and ADDRESS of your past employers, obviously there is not nearly enough room to do so in that specific field.

This form example on ***removed*** seems to be flat out WRONG since he does not provide employer's address: http://www.***removed***/forms/g-325a-sample-biographic-information.pdf

I will attach a sheet to the back with that info but I'd like to know your opinion on this issue...

Yes that's what you are suppose to do if you need more space. I used extra sheet for G325 and I-130. No issue.

Yes that's what you are suppose to do if you need more space. I used extra sheet for G325 and I-130. No issue.

I'm in the process of reviewing my package. My wife has 4 names, hence 2 middle names. So it won't fit at the top of the G-325A respective field, I left it blank and it's on the attachment, but it seems a bit off putting to leave it out of the form itself, I noticed I could carefully write it in the box in a way that one name would on top of the other. Example:

Last Name Given Name Middle Name

BANANA ANA HANNAH

MONTANA

Is this structuring OK?

Timeline

We successfully used the same technique on our G-325A and I-129F as dwheels76 previously mentioned - Use a supplemental sheet. This technique is specifically mentioned in both of the primary petitions (I-130 & I-129F) instructions.
