So me and my fiance have started working on the files for her green card. I am a little confused about I-693 form. It is the medical exam form. She had her medical exam back in October so it should still be good. But the instructions say to include a vaccination supplement when filing it. So how do we do this?

Do we go see a civil service doctor and do another exam?

How can we receive a copy of the vaccination supplement from her previous exam?

Thanks in advance for any advice.

Did she in fact get vaccinations during her exams or not? Did she have a sufficient vaccination record otherwise? I was under the impression there were shortages in October and also at the IC location they were suggesting people to 'just get them in the US' they werent needed. If she doesnt have it with her and already here in the US....i'd think better just to goto US civl surgeon and call it a day

We went to the civil surgeon, which is a racket. Cost us ~ $500 or something around there. I think regionally/locally it may be better elsewhere. YMMV

Otherwise while there, you preprep and fill out most of it, they'll fill out the rest, give you a copy and a sealed copy.

If she had a completed DS-3025 she does not need a I-693. If she does not have a completed DS-3025 she will need to seek a civil surgeon. My wife did not have a DS-3025, she had her shot record. We took a copy of her shot record to a civil surgeon and they transposed those to her I-693, plus the shots she was missing.
