I-130 PETITION HELP!!!

hi guys, when filing the I-130 and I-485 concurrently to adjust status of my F1 spouse to a permanent resident, In filling form G325A at the bottom section before the part for signature which states " this form is submitted in connection with an application for?" which of the boxes do i check as the petitioner and what box should my spouse check when filling the same form? is it the status as a permanent resident box or I should check other and specify form I-130? does the g325a biographic form need to be submitted with i-485 application also or just the I-130? if two biographic forms of my spouse has to be submitted each with I-130 and I485 that brings me to the first question about what boxes to be checked in relation to the application submitted. Would appreciate your answers.
