Using James' Shortcuts I created the barcode sheet for the I-864 Packet. After "adjustment" using MS Word the result is a very long thin barcode. I sure hope it works!

The "actual dimensions" end up being a little under 5mm tall and 83mm long.

That's a little over 1/8" tall and 3 1/4" long.

Can someone give me the measurements of the one sent by NVC?

a barcode is a barcode, it'll work no matter how big or small.

the barcode sheet is printed on standard size copy paper.

truth be told, though, the barcode sheet isn't as necessary as it implies. just make sure you reference your case number on the documents.
