The sample form here http://www.visajourney.com/examples/Form-I-134.pdf leavesthe bank account and financial assets section blank. Is that because the income itself is sufficient?

If I get the letter from my employer now but the I-129 is not approved for 6 months, is that letter stale? Or does the letter plus recent pay stubs from the same employer work?

Is the employer letter addressed To Whom It May Concern? I do have a letter addressed to me giving me my last merit increase and salary. Is that sufficient for the I-134 (with pay stubs)?

FYI, the current I-134 form has an OMB expiration date of 02/29/2016. Since this sunset clause seems to be new, I wonder if anyone knows how it impacts in flight applications?

Other than the sunset clause, it is identical to the one in the example.

Address the letter To Whom It May Concern is fine. Mine included date of hire, Job title, and hourly wage on company letterhead. It also stated I was a full time employee and was a few months old by the time of interview. Not a problem. Just send recent pay stubs. If your income is over the poverty level requirement there is no need to list assets it bank account info. At least I didn't.

Just like LionessDeon stated above, but you don't need this until the interview in Kiev. After you send the I-129f package, file the forms with the IRS to get your tax transcripts for the last 3 years, they take a while to come, a good month if I remember. Once you have the interview date set, get your employer letter and latest pay stubs together so they are current. If your income is adequate (125% over poverty level), the other assets are not necessary to fill in, I did not list any. If you do need to list the other assets, you will need to get proof of values, account balances, etc. Just check for the latest I-134 before you take or send the package to Ukraine. Good Luck.
