what documents and/or ID do you have that shows the proper spelling ?

and what block of the I-90 are you having problems with ?

or do you need a human to fill it out for you?

Sometimes my language usage seems confusing - please feel free to 'read it twice', just in case !

yes i need a hand filling up the i-90 as im really nervous filling up paper works. i just printed out the i-90 form and please correct me if im wrong, the part that i needed to fill up are:

part 1: information about me, my full name, mailing address, physical address and additional information. should i skip the part that ask about the name exactly printed on my permanent resident card?

part 2: check the LPR part and section a as my existing card has incorrect data because of DHS error

part 3: processing information would be just 3.a and 3.a.1 right? and should i still answer the number 4 and 5? and should i still answer biographic information as well?

part 4: should be skipped on my part

part 5: should just get a check mark on 1.a ? then write on applicant's contact information (3,4,5) and write my name and sign it under acknowledgement of appointment at uscis application support center?

and part 6-8 should be omitted if i will fill up the application myself.

sorry if i have it written like this but im really lost with this paper work

Fill in the blanks on the I-90 form. The proof you will submit is your BC and a copy of your bio page from your passport and copy of any other official documents such as marriage licensee/certificate. Three different pieces should satisfy them. Make sure you send it with tracking. Also, include a cover letter explaining why you are making the request. Don't send your old card unless specifically stated in the I-90 to do so. I would include a copy of the GC with your submission.
