Power of Attorney for minor child's passport application

A pleasant day to all! I am a US Green Card holder petitioning my 11 y/o daughter in the Philippines. I have just recently received our approval notice (I-797) for her I130 petition. The requirements for her to apply a PH passport is a Power of Attorney since she will be applying without me over there and is under the care of my parents as her legal guardian.

What is a Power of Attorney and how do I get it?

Is it a form I can download online, fill it up and send to her?

Do I have to go back to the Philippine Consulate (Chicago) and get it there?

Hi! just go to any Notary/ Attorney's office in the city where you live and ask them to make you one. They have a standard format for that. You will sign it and they will notarize. Not a US standard form, not from the Phil consulate office.

It was just a statement that you authorize your Mother to accompany your son to process his documents and sign when necessary on your behalf. You may wanted to specify for Passport Processing, Medical Examination, Visa application / Interview, etc. With this you don't need to make another SPA for the next processes that your son needs to do.

SPA should be original, ask the lawyer that you need several original copies and sign all of it. Send several original copies to your mom.

Jan 31 - received USCIS letter/ Notice for interview 

March 3- Interview, approved on the Spot

March 8 - received GC

Dec 2018 - To file ROC
