Hey guys, I'm slightly confused, but I apologize if this is a really simple answer...

So I'm looking at putting together my adjustment of status pack now, and I am looking at the form I-693 Medical Exam. However, I noticed in the step by step here, it says I DONT need another medical if my UK one is within a year - which it is. But so I was wondering, what do I file instead of the I-693? Do I still need to find a doctor in America to fill out the I-693, or do I just send off the vaccination record list thing that Knightsbridge Doctors gave back to me after my medical? Just a bit confused what I'm meant to send off for this one!

Hi -

Yes, you still need to submit the I-693. You need to submit parts 1 (personal info), 4 (civil surgeon info) & 7 (vaccine info). The civil surgeon will transcribe the vaccination info onto Part 7. We called around and found a civil surgeon who did the transcription section and charged $25.00.

I-751 Removal of Conditions Timeline:

3/19/2018: Sent Application to VSC

3/20/2018: Application Packet Received By VSC

3/26/2018: Check Cashed By VSC

3/27/2018: Received NOA Letter w/Notice Date of 3/22/2018 

5/11/2018: Received Biometrics Letter

5/21/2018: Biometrics Appointment 

5/30/2019: Received Notification New Card Being Produced

6/5/2019: Received Green Card

NO you do not have to see a civil surgeon if you got all the shots needed marked on your DS-3025 form at Knightsbridge. I wrote something for the AOS forum that is pinned at the top. Thank you so much! So I've read it through and understand that basically I just send what Knighstbridge gave back to me. When I attended my visa interview, they also gave me back a CD with my chest x-rays on.... should I send this too just to cover all the bases or will the DS-3025 suffice?

No. You never need that CD for anything. They just give it to you or would throw it away. A doctor read it and the results are on your medical report (that you are free of TB.) immigration does not interpret x-Rays. They contract doctors to do the medical evaluations and write the results on the forms.

You send a photocopy of what Knightsbridge gave you if it shows you were covered on the required shots (as explained in that thread.) Don't send off your only copy of anything.
