Hi All,

I am quite confused regarding the filing of I-693 FORM.

On the Instruction, it says it has no filing fee... However Ive read in some other forums here that the your local civil surgeon will charge you?

Charge for what?

I have already completed my medical exams (blood test, etc.) including all required vaccinations (TDAP, MMR, IPV and all Booster Shots) before my k1 interview. I still have the copy of my medical exams of my k1 from an approved and referred hospital of the US Embassy in UAE where my interview took place.

Is there a fee that I need to pay the civil surgeon just to sign and approved my medical papers or do I need to do another set of medical exams again? (I hope not) :( If so, what are the medical tests I should take again for a civil surgeon to sign my I-693 form?

A civil surgeon can charge you whatever amount they want for their services.

Hi thank you for this helpful link.

Does the booster shots matter? on my DS-3025, the booster shots were not stated as my booster shots happened after the interview and after my visa was approved. So instead, the local approved hospital gave me a separate booklet which contained all of my recorded vaccination shots including the recent booster shots.

So I was wondering should I still see a local civil surgeon and ask him to credit my booster shot on the i-693 or it doesn't really matter?

You only need one in a series for AOS, so if you have each required vaccination marked off on the DS-3025, then you don't need the civil surgeon. If you received some of the required vaccinations after the K-1 medical and they are not marked off on the DS-3025, then you need to find a civil surgeon to transcribe your vaccinations onto an I-693.

Hi All,

Charge for what? For transcribing the vac records onto the 693. They are free to charge whatever they want, so shop around.

Is there a fee that I need to pay the civil surgeon just to sign and approved my medical papers Yes, or do I need to do another set of medical exams again? Many CSs will try to get you to redo the entire medical, so shop around, you want one that will only transcribe the vac records to the 693.
