No that is wrong. The Given Name is your last name. Then you have your first name and middle name.

You need to contact USEM to have this corrected or at best have them explain what needs to be done.

Everything is fine and as it should be. Given name is your first name and the embassy always list your middle name with your first name under GIVEN NAME.

No that is wrong. The Given Name is your last name. Then you have your first name and middle name.

You got the backassward... given name is first name not last name. SURNAME is the last name. GIVEN... as in the name given at birth. ;)

Everything is fine on the visa, middle name is ALWAYS listed with the GIVEN NAME (first name)
