Hi everyone,

We have assembled the the I-485 package pretty well, with a cover page and separate table of contents for both my daughter and I, as well as my husband. I am pretty confident I have everything right and in order.

HOWEVER, I am so confused about the I-693 and can't seem to find any recent information on here about it (unless I've totally missed it).

Do I need to include this in the I-485 package? I know that for K1 visa's, you don't have to redo your medical as long as it hasn't expired (we are no where near that yet) but, we still need to provide proof that we are up to date on vaccinations, etc. This can apparently be done by sending in a copy of the DS 3025, which was supposed to be completed at our K1 medical before the consulate interview.

But, I don't have it! I was never given a copy or even told about this form, either from the Doctor or the consulate.

So, my question is, can I file for AOS without the I-693? Does the USCIS already have the DS 3025? If not, how would I obtain the DS 3025?

Any advice would be greatly appreciated, and I apologize if this has recently been asked by someone else!

Thank you. I know I am up to date on everything, but I have no idea if a DS 3025 was even completed. People have mentioned it gets handed over when you enter the U.S, in the secret envelope we are told not to open, and then forwarded to the USCIS. Is it possible that is what could have happened?

If it did I have no idea how you would get a copy, hoping someone else could chime in. It would be an awful shame if you have to take all the shots again and pay the civil surgeon. We had to see a civil surgeon because my wife was missing a few vaccines and they just transferred her already done vaccines over to her I-693 at a cost of 450 bucks.

We were in the same situation when we did our AOS. We did not have the DS 3025, we were told it was sent t USCIS and would be in our file. We took our chances and just mailed our AOS stuff. We were never asked for an I 693 and never got an RFE. I've heard others that had problems and had to get the I 693 filled out by a civil surgeon. When my husband got his medical in his home country he asked for the DS 3025 and the doctor refused to give him a copy.

Interesting! Maybe that is what happens then? It is directly sent to the USCIS. and maybe the lucky ones receive a copy! lol. and some i have read get lost, or are unknowingly incomplete :S

When I picked up my medical exam, they handed me the sealed envelop to give to the consulate (DS 3025), a CD of the X-rays (for me to keep) and the I-693 filled out by the doctor. So, you did you get the I 693? It was just one piece of paper that I easily could have misplaced.

Just want to point out that there would be no I-693 from a K-1 medical exam. The panel physician abroad uses DS(Dept. of State) forms. Only a civil surgeon here in the US uses the I-693 form.

You usually get a copy of your DS-3025 from the panel physician, but if not, yes, it will be included with the rest of your medical documentation in the sealed envelope you handed over at your POE.

If you are positive that you got the needed shots, then actually mailing in a DS-3025 photocopy is not necessary. The original form is with your other medical results and was turned over to USCIS at POE. That's actually the one that counts because it remained in the "chain of custody" so you didn't have an opportunity to alter it. The photocopy isn't really an official copy and is more for a reminder that you got complete at your visa exam. Of course without having a copy, you don't have the opportunity to see if the rest of the form was completed correctly.

our medicals were done in Toronto and then mailed straight to the consulate so that we wouldn't have to travel back to Toronto to pick up the results. the doctor didn't give me anything except a receipt for my cash payment.

SO, i guess from what everyone seems to be saying, it is safe to say the DS-3025 was probably in the sealed envelope i handed over at my POE, and that USCIS should hopefully have it.... emphasis on hopefully!
