Ok, so the question has probably been asked a million times, let me make it one more.

When you do AOS from a K3 visa, one of the things you have to submit is the I-693 vaccanation form. This vaccination form is supposed to be signed by a civil surgeon, and sounds like it is just a transfer of information from the K-3 medical exam to this USCIS approved form.

My questions:

1. Has anyone ever printed the PDF copy of this form, taken it to the civil surgeon who did the actual medical exam in the foreign contry and have them complete it? Is this acceptable or are you supposed to get a civil surgeon from the US instead and line their pockets with $50-$200 instead?

2. Is it supposed to be in a sealed envelope?

3. Are you asked for the orginial medical exam and if so, I thought they took it from you at POE. Has anyone asked the POE officer to give them their exam?

1. It has to be done by a Civil Surgeon here in the US

2. Yes must be in a sealed envelope (ask for a copy)

3. Original medical is in the sealed packet that you hand in at POE... you could be asked to have it redone if its more than 12 months since you had it done or if USCIS lose it.....

1. Not me. The Civil Surgeon I went to knew exactly what I wanted and had the required form.

2. Yes. I got a sealed envelope a week after I saw my CS. She also gave me a copy for my own records. I submitted the sealed envelope with my AOS application.

3. I gave my CS a copy of the DS-3025 worksheet that the embassy-assigned doctor in Singapore completed for me. The doctor told me to make sure I kept my copy safe and to make copies of it for the rest of my immigration process in the US.
