Hi guys, I'm new here. Nice to meet you all!

Ok, so I came here on a K-1 visa from the UK, got married, filed for adjustment of status in November 2014. I have had a few correspondences from USCIS since then, all asking for various bits of the same information I already provided. I have noticed that they seem to ask for the same info over and over again.

Anyway, I recently received a letter from them AGAIN asking for more info... the info I've already provided them. There's just one snag, however: They're asking me to send them the i-693 form (the one the civil surgeon fills out after your medical exam). There are two problems with this, the first being that the civil surgeon I went to is in London and the second issue is I don't have any copies of the form to send them. I have only my vaccination records and a copy of my chest x-ray. The original i-693 form was submitted to them over a year ago upon entry to the US, so I'm assuming they've lost it? They gave me a deadline in which to send them the information, but it's coming up in about a week and I'm stressing out like crazy. If I get deported due to an admin error I will be so angry and upset... I feel like everything is in danger of being ripped away from me.

I went thru the same issue with my wife's AOS, unfortunately, you will need to do the same test in the USA. here in NH cost us around $500. after the we sent the information, they approved the case in less than 2 weeks . my guess is that you should call the immigration service center and ask for an extension on the dateline, and check where the nearest doctor. use this link https://my.uscis.gov/findadoctorand do yuor appoinment as soon as possible. good luck

The doctor in London is NOT a civil surgeon and did not fill out an I-693. It didn't get lost because you never had one. The error appears to be your own possibly.

What was the date of your medical at the Panel Physician (Knightsbridge Clinic) in London?

What date did you submit your AOS packet?

What is the exact wording of the request? Type to out.

The Adjustment of Status is a different process. You will need to submit some of the same information as you did with the K visa. But they don't mix the files. So nothing was lost. A new file is being built and you need to supply what is requested.

When we did the Adjustment of Status a copy of the vaccination record (given by the doctor who did the I-693) was accepted and nothing else was needed. However that is not always the case.

Hi guys

Many thanks for your replies. I have since sorted this and it is solved.

It turns out the i-693 form is only valid for a period of one year, and since my application has taken longer than that it means the original medical exam had expired and I was required to attend a new one so USCIS could update the case.

The original medical, if you are speaking of the K-1 medical abroad, was done by a panel physician, not a civil surgeon. There was no I-693 with the K-1 medical. The K-1 medical includes Dept of State DS forms that would have been included in your sealed envelope that you turned in at your POE. If the K-1 medical was over a year old, then the adjudicator may ask that you get a new medical and provide an I-693 from a civil surgeon within the US.
