ROM Affidavit of Delayed Registration Signatures

I've been trying to call the PH embassy in LA for a WEEK now. I call at least 5x a day and nobody answers the dang phone. Feels like the motherland all over again.

Anyway, the only questions I have for them are:

1. Do both the husband and wife need to sign and notarize the affidavit of delayed registration or will the Filipino spouse be enough? We were married 2 years ago and just now registering.

2. I'll probably have the ROM mailed to us. We're only gonna be in SoCal for a few days for a vacation and I don't feel like making 2 trips to the embassy (to drop off the docs and pick up the ROM). What size self-stamped envelope do I need to include in our packet?

3. Do we need to sign the ROM application form there in person?

I hope someone can answer these. We leave on Wed and I only have today and tomorrow to prepare the documents. Thanks!

Also, the website doesn't say that we need to bring passport photos but in other threads I read that it is a requirement. Is it no longer required?

1. Both husband and wife need to sign the affidavit of delayed marriage form. It needs to be notarized only if you are mailing it to the consulate.

2. Take a legal-size envelope and an 8 1/2" x 11" envelope with you and ask the consulate which will work best.

3. To play it safe, you should sign the form when you are there.

The consulate web site doesn't mention whether passport photos are still require. However, it's the Philippines Consulate (where anything can happen), so you had better take passport photos with you just in case. Or, you likely can get passport photos taken at the consulate.

Thank you! I just downloaded the affidavit and left it blank for now. Would you know how many copies of two affidavit will be needed?

Thank you! I just downloaded the affidavit and left it blank for now. Would you know how many copies of two affidavit will be needed?

I don't know, but I'm pretty sure the info is on their web site.

Just to update.

They didn't ask for photos.

They needed 4 copies of the affidavit.

They accepted the affidavit with just my signature, notarized.

We signed the ROM app form in front of the officer.

All in all, it wasn't as bad an experience as I expected it to be. We got there like 5 minutes before lunch break and I was convinced that we were going to get turned away (you know how we Filipinos are with our lunch breaks). The lady was mataray when I first got there. I begged her to wait until my husband got there (he was parking the car. with our toddler. in the pouring rain). She was like "ummmm... you have 5 minutes". So I went to another floor to get copies of some docs. Ran back and she was gone. LUCKILY, the guy in another window helped me out and waited until the husband and child (both dripping wet) arrived. I had a self-addressed envelope with me and I just asked them to mail it back.

We were there about 15 minutes total. I wish I'll never have to go back there ever again.
