Hi,

I have answered Yes for questions 15a & 18 and have my military service listed in Part C. Given that the answer for Part C is basically the explanation for 15a & 18, do I still need to add an extra sheet to explain the two yes answers?

From the form itself, "If your answer is "Yes" to any question, explain on a separate piece of paper."

So, your answer is yes to 2 of the questions, therefore you would need the separate sheet explanation, as it instructs.
