Good evening everyone. My fiance and I went to SSA last week Thursday to apply for his SSN, the card was sent in the mail.today but it reads "Valid for work only with DHS authorization" is this a normal thing or?

2. On form I-485 (permanent residence) page 2 part 3B where it says "list your present spouse name. I am the petitioner(lady) will I write my Father's last name (family name) or my husband's last name (family name)?

3 On form I-765 (employment authorization) question 16 I put C(9) is this ok?

4.On form I-131 (travel document) page 2 part 2 I clicked on 1(d) is this ok and am I going to write the intended date of departure, lenght of trip, purpose of trip on page 3, and write on a separate paper for part 7 on page 4? Even if he has no intention of traveling abroad while his AOS papers are pending? What can a genuine reason of visiting abroad be?

5. On form G-1145 (e-notification form) am I going to write his last name/mine

E.g if my last name is Jones, and his is Williams. Am I going to write Jones/Williams,because it says applicant/petitioner full last name, full first name, and middle name? I mean is it going to be his information or mine as the petitioner or his as the applicant?

6. Form G-325A (biography form) are we both going to fill it out or just only him adjusting his status? Tnx so much all.for your responses

2. On form I-485 (permanent residence) page 2 part 3B where it says "list your present spouse name. I am the petitioner(lady) will I write my Father's last name (family name) or my husband's last name (family name)? Did you take your husband's name when you married? Put what your name is now after marriage.

3 On form I-765 (employment authorization) question 16 I put C(9) is this ok? Yes, that is the correct category.

Date and length can be left blank and the hand-write Unknown for those after printing out the form.

Part 4 is not filled out. Skip to Part 7 where it tells you to on the form.

No supplement needed.

His name is filled out on all the forms. They are all his except for the I-864 the USC petitioner/sponsor needs to provide. The immigrant is the one filing for AOS. He is the applicant.

6. Form G-325A (biography form) are we both going to fill it out or just only him adjusting his status? Just the one filing for AOS.

Tnx KayDeeCee, we have not married yet will do that next week, we are just filling out the forms prior. Pls, do you mean even if I"m married I still have to use my Father's name? I don't get this can you explain more for question 2?

When you marry, you will choose on your own what name you want to use after marriage. That is your choice. You put the name you will choose to use after marriage, though for the USC petitioner/sponsor, it doesn't matter all that much. You could still list your maiden name on the AOS forms, and change your name over at your own pace, if you choose to.
