On the VWP, the limit is 90 days. Staying under 3 months is meaningless since that is a requirement to use the VWP.

No one can tell you how many times. It will depend on your particular situation.

There's a big difference between a person who comes once a month for a few days for 12 months versus a person who uses the VWP twice for back to back 90 days. Which one looks like he is abusing the VWP to live in the US?

If it looks like you are abusing the VWP to de facto live here with brief exits, then you could be denied entry and your VWP privileges.

ESTA has nothing to do with how many times you can visit the United States in a year. It's a pre-clearance background security check form that only needs to be filled out and renewed ever so often. Under the VWP, ideally you need to be spending more time in your home country than you are visiting here. It will always be up to the IO to decide if you can enter, regardless.
