Dr did my DS-3025 wrong

Got everything sorted with my AOS now just need to sort my DS-3025 and i-693.

So when I had my medical done in the UK, The doctor doing the vaccinations put down that I needed the tetanus, diptheria and polio vaccination, at a later date I went to a nurse to get that done and she pointed out (correctly) that I'd already had them in 1994 and 2009 so I do not need them. However the envelope I handed over at POE obviously contains a form saying I need to get that vaccine done. I'm going to get my flu shot done by a civil surgeon soon, I don't want to get another DPT shot. Can I just bring him my UK vaccination records, get the flu shot and have him transcribe the correct information?

Also I have another question. On the letter I got from my medical (Which was done in March) It says it will be valid for 6 months. I have read older posts on this forum that said I don't have to get a new medical if I had one done within a year. Am I OK to just get my vaccinations? or am I going to have to get a new medical done?

Is there anyway you could post a photo of your DS-3025? You may be fine and aren't interpreting it correctly. Civil surgeons can gouge you for a lot of money.

Yes, as long as your medical was within the past year, it is okay. Read that in the I-693 instructions. The six month thing is for visa travel.

I attached my DS-3025 and the letter they gave me. I think I have to see a Civil Surgeon anyway due to the flu shot. I'm hoping it shouldn't cost too much with just a flu shot. I'm not sure exactly how it will work though, will they give me a new DS-3025? What exactly is the protocol if you've had the medical but are missing a vaccine?

(The date on the DT, DTP, DTaP is incorrect. My latest was in 2009. I'm hoping if I take my medical records to the CS he can transcribe it correctly when I get my flu shot.)

Yes they did make a mistake if they did not copy down your 2009 shot from the records you took to them. It is not tetanus, diphtheria, polio. The P is for pertussis. If you show the 2009 record to a civil surgeon, he can copy it it to a new vaccination form found as part of the I-693. Before I was thinking your March exam was not in flu season and wanted to see if they had marked the waiver code for the flu shot. Flu season doesn't actually end until March 31, so you wouldn't have gotten a waiver. You can get a flu shot anywhere and also take the proof to a civil surgeon to put put on Form I-693. (Note: DS-3025 is only done abroad. Once in the U.S., it is Form I-693 that certifies your vaccinations.

The protocol is that a civil surgeon does not have to do a whole exam. He verifies your identity and fills out the vaccination portion of the I-693. The problem many find is that civil surgeons will insist that you need an entire exam and want $XXX.xx to do it. Keep calling until you find one who will not do an exam. Ask the price on the phone for certifying your vaccinations only without an exam. Some make the mistake of just asking about the service and when they arrive, find out the charge is several hundred dollars.
