On the form that asked you about "Application residence last 5 year", we are able to put the last 5 residence that can fit on the form however, it does not equal to 5 years. Do we have to create another sheet to make it 5 years at least of residency or would this be sufficient. Please see sample

If it does NOT add up to 5 years you must add an attachment sheet. List the question and it's number, then add the information on the extra sheet of paper. On the G-325a write down "see attachment" on the last line.

What if you can't recall a past address or exact times.

You do the best you can. There is always paperwork from the past etc to help you figure these kinds of things out. You just have to dig a little.

The DS-260 will required you to list every address you've had since the age of 16. So I suggest looking deeper into previous addresses.
