I called a doctor that is from the area that is on the civil surgeon list to see if she would be able to help us with his

(I-693a). and she then told me that she is a ped. doc. and she pointed me to another doctor that is in the same clinic. however we got talking and she told me that they don't supply the forms. that we r to be organized and have them when we come in. and that a civil surgeon has to do the full exam before signing any forms.

what do i do, i can't find the I-693a form... and god help us thats all we need is more dang bills!!! LOL

There are civil surgeons who know that a full exam is NOT needed if your medical is less than 1 year. It takes time to call around and find one. We had to travel over an hour to the one we found. They had the I-693A that we needed, filled it out and gave my husband the shot he needed for $50. I don't think there IS an I-693A form to be found...I think it's supposed to come from the CS.

Hi! To find the I-693 document, go to the USCIS home page and click on Immigration Forms. You'll see it there.

Yes, that IS the I-693 for the complete medical. However, if she doesn't need a new medical all she needs is the Vaccination Supplement and only the civil surgeon has those.
