I'm doing this for my spouse I am American Citizen and he's from Ghana. Here's my question on completing form, 1. Family name is this my current name as I carry my spouse last name? 15. Employer- is this hisbor my employer? 17. Spouse and children is this myself and do I have to list my kids though they are not my husband's? And if so I saw somewhere it showed only kids 21 and under?

Please read the form thoroughly.

Family name is your current surname.

Employer is actually on the beneficiary side

Spouse and children of your RELATIVE. So you list yourself and any children your RELATIVE has. If your children are not also his children, you do not list them.


Also if you are having issues, please search out a good immigration lawyer to help you with the forms.
