Request for Initial Evidence (RFE)

I am the petitioner (filed for my parents) - i received an RFE for both with this request:

Form I-864, Affidavit of Support, was submitted; however, Page 3, Part 4 of the petitioner/sponsor's Form 1-864 list someone other than the petitioner/sponsor or Page 3, Part 4 is blank.
Form I-864 Affidavit of Support was submitted from a joint sponsor; however the names listed on Page 1 and Page 3 are not the same.

when i check - the names are the same except page one has my first name, middle and last and on page 3 it specifies to list Last, First and Middle names in that order (in fact your are given boxes for each)

I am confused - what am i to do different? Should the first page read Last Name, First Name, Middle Name in that format all in one line? Has anyone ever encountered this before?

any help will be appreciated.
