You need to file the I-765 with your Adjustment of Status. You are no longer eligible to file based on your K-1 status, since your I-94 is expired.

When filing with your AOS, you are category C 9 -- Adjustment Applicant.

K-1 is your current status.

While trying to answer question I'm also searching for some sample wherein it made me more confused.. Since I'm married and I'll be using my spouse name I should write my spouse family name for the question 1. FULL NAME ? and my middle name would be my family name before marriage ? How bout the OTHER NAME USE (INCLUDING MAIDEN).
