Timeline

So we are about to send off our AOS in the next few days.

When I had my medical in London, they told me that when in America, I would need some additional vaccines. I agreed and proceeded with my interview and am now in America and had my vaccines done at a very expensive civil surgeon who gave me the white envelope which includes my i-693 form, which I am not allowed to open, only the USCIS staff can.

I am just wondering, shall I send my London exam results which have marked down that I don't have the required vaccines, or shall I not include the London one and only include the one from the American civil surgeon, which has my full vaccine checklist complete, or shall I send both and risk confusion for the USCIS officer?

Also, can anyone else from the UK who had their medical shed some light onto what documents London Knightsbridge gave them? I have a CD, and a vaccine checklist...is that really all?!

I would send both the new envelope, and the vaccine checklist you got from Knightsbridge (they probably do not need the London one, but no harm in sending it just to be sure). You can include a note with the London checklist stating that you have enclosed a new and completed I-693. All you get from Knightsbridge is the CD, and vaccine checklist, the full report was sent to the embassy.

You do NOT need to send anything from Knightsbridge. Your new vaccination form supercedes the incomplete one. Your complete medical file from Knightsbridge was in the envelope you turned in at POE and will end up in your AOS file. It looks similar to this, but this is an older version. http://photos.state.gov/libraries/vietnam/8621/pdf-forms/DS-2053.pdf You got a photocopy of the immunization sheet but all the originals are sent to the embassy then USCIS.

The chest x-ray is yours to keep or throw away. It's not needed for immigration.
