I just called the civil surgeon's office to check on the status of my hubby's vaccination supplement (he came on a K-1). And she said that she would get the doctor to sign it and have it "packaged." Do the supplements usually come sealed in some kind of package? Or is she getting this confused with a regular medical exam? Or am I just confused today? :wacko:

Am I the only one who thinks this (the sealed enevelopes) is very "cloak and dagger" and totally unnecessary? So, I guess I will get in trouble if I open it. Because I have a feeling it may already be sealed as I type this. :crying:

I am sure it will be ok though. We all must have positive thoughts. I will just have to make sure we get a copy someway, somehow. They will likely have a copy in their files even if the envelope is sealed.

I received mine in a sealed envelope as well, but they made a copy for me beforehand (I didn't even have to ask for it :D )

They refused to give me a copy of the suppliment. They did however show me the documents before she sealed the envelope, stamped and taped the seal closed. After the hassels of getting the suppliment without the full medical like all the CS I called, I didnt have the energy to complain or make a fuss.

Yes, I am a little tired of explaining over and over again what my husband needs and listening to outrageous price quotes. In fact, I decided to drive to another state to get his vaccination supplement done because the first place in town I called whoever I spoke to was so incredibly rude and the second place where we live seemed not to understand what I was saying at all and quoted me a price "in the 200s." So, I drove on down to my hometown for his appointment because they were nice on the phone, seemed knowledgeable, and quoted what I believed to be a fair and reasonable price ($10 to copy shot record, $45 for the blood titre, and $20 for the Td shot). That price was less than rude lady who said $95 but it would probably be $140 even though I had to pay for gas. Now, I just have to drive back to get the package. But, at least it's done. Yippee!!!

I know how that feels. The CS's I called said that they couldn't give me the suppliment without a medical done, and I turned blue in the face trying to explain, that I didn't need a new medical, I had one abroad before and as part of me getting my K visa..but to no a vail, they said full medical at $200.00.

Finally I got someone who was sort of willing to listen to me explain the situation. She agreed to let me fax her my Work Sheet, reciept from my medical abroad, and copy of my visa. I included and circled in a thick black line where it stated on the AOS instruction form that K's don't need a new medical, just a vacc suppliment...and YAY, she called me back and agreed to complete it for me. $50 each for my daughter and I.

Oops never mind "$10 to copy shot record" but get a copy of the supplement form filled out by the civil surgeon.

I did ask for a copy, they said they are not permitted to. I stated that in my first post on this thread.

Here are a few web pages that deal with the vaccination suplement.This one is about the Civil Surgens Instructions from the CDC,It clearly states that they are to give you a copy of the I-693 Supplement.Page 2 under Final Action.

Everything we have done was without the help of an attorney.We have not had any problems thus far and will not,I believe.

Glad I didn't have problem telling them that I don't need a full medical, My supplement came in an envelope with the stamp on it but it wasn't sealed, so I made a copy myself and sealed it. But I already knew there's no problem with my vaccination cuz the doctor filled out the form in front of me :) though she had to take my blood to make sure that I'm immuned to chicken pox.
