Each embassy handles things a little differently, but YES go ahead and start filling out that DS-160 and save it. Then pay your fee. Once you have a receipt number that you paid, you should be able to enter on the DS-160 and then schedule your interview.

At least that's how it worked for Ukraine.

Are you going for a K-1 visa? For K1, you should complete the DS-160 and the Embassy in Islamabad will schedule the interview for you. They will send you and interview appointment letter (via American Express) and you should get your medical done before the interview. My fiance said the doctor in Karachi required the medical to be done at least 14 days before the interview. So make sure to schedule your medical as soon as you get the letter.

Did you not get the packet 3 yet? How long has it been since your case was sent from NVC to Islamabad? They should have sent an email within a couple of days.

I (the petitioner) received an e-mail from the Embassy in Islamabad about a month after our petition was approved from USCIS. It was probably about a week after it was received in Islamabad. It contained information regarding Packet 3.5 and instructions on how to send our information in. I ended up forwarding the email to my fiance because he didn't receive anything. The Embassy in Islamabad will schedule an interview for you (we're still waiting on ours). Good luck!
