Is it okay to schedule the medical and fill out the DS-160 before we've received the packet 3 email from the embassy (using the case number from nvc) or do we need to wait for the actual packet?

It depends on the embassy. Mine (Montreal) specifically advised against this. In fact, without an interview date, the doctor's office would not schedule someone for a medical.

I would recomend to wait to get the email from the embassy in San Jose with the instructions.
