Medical exam and extension

I was approved for my I-129F fiance visa petition. My fiance in the US paid for a lawyer to help us, she hasn't been much help these past couple months. The NVC sent my paperwork to the US Embassy August 5th, 2015 and on ceac.state.gov/ website says that I'm ready for my interview.I filled out my DS-160 and my visa interview profile as much as I could. It stopped me at the receipt part and would not take my receipt number from the bank. I am having troubles talking to the correct person on the phone at the US embassy, and I believe I need an extension. My case expires November 12, 2015, which with the weekend is technically in 4 business days. Any advice on how to talk to the correct person for the extension? Also I still need to get my medical exam, I have read that you do that when you schedule your appointment, but i have also read that it takes a couple days for some results to come back. Should i go ahead and get my medical exam done? I found online what doctors are approved by the US Embassy in my country. I also live roughly 6 hours away from the US embassy and where the doctors are located that the US embassy uses.

the medical exam usually gets done about 2 weeks before the interview, i suggest you dont schedule a medical exam appointment before you have an interview date.

I see everyone also talking about receiving packets. I have not even received 1 packet, what does this mean?
