What is the correct way of filling up the online NBI form?

The instructions from the USEM website states that clearances should be in the applicant's:

1. Current Name

2. Birth Certificate Name

3. Maiden Name

4. Any Aliases or nicknames used including different spellings

For those who are married, what should I put in the following:

1. Last Name

2. Middle Name

And do I need AKA/Other name for my maiden name or married name?

You must use all names that you have ever used. Best rule of thumb is use what is on official documents like your birth and marriage certificates.

Thanks for the quick reply. I'm confused as to what I should put on the last name and middle name because when I called USEM, the operator told me that I should put the current name which is my married name but the guard at the NBI Clearance center told me that it should be in my maiden name.

If I follow the instruction of the guard, should I put my married name in the AKA even if the "husband's surname" is already stated there?

Also, some said that the "first name-mom's maiden name-dad's surname-husband's surname" should be included. But I never used that format in any of my documents. Do I still need to include that format?

Use your current name as listed on your marriage certificate. Then list all your aka's like your maiden name keeping to the format as shown on your BC and any other names used.

Yes you need an AKA for other name. If you are using your married name on the NBI then you AKA your maiden name.

Current name: The name that you are using now, so if you are married that should be your Mrs. Name

AKA: Your name when you are single/ maiden name; and if you have used any other names before; i.e previously married and re-marries.

Once you reach the photo capture in NBI the encoder will clarify your name, will ask you to review the details. Ask questions, they normally edit the name from what we encode it on the online application inserting Y on the names.

Ex: AKA: Delacruz, Juan Y Crisostomo

Do the online application and you can also pay through online banking or any payment centers. I did mine 2 weeks ago.

Hi. There won't be an "AKA" written on the NBI clearance itself. I know because it is what we have currently. If you are a married woman, Your maiden name, married name, middle name, first name is what you need (unless you have spelling corrections, other names used on official documents, that is what is considered as "AKA")

When you go to the NBI office, they will reconfirm it anyway to you and will make the necessary corrections.

I have a picture of the NBI Clearance if you want to see whta it looks like.

I saw your other post.

Your wife filled out the NBI application using her maiden name? Why?? Is she not using your name legally? Is she applying a visa in her maiden name?

If she is using your name: on the Form her family name should be your last name, middle name is maiden name, Name

on the NBI form there is an portion other names/ AKA: this is here she should write her full maiden name: Ex. Maria (first name) Aguinaldo (middle name) Cruz (maiden name)

I am 100% sure she didn't write anything on the form on the other names/ AKA that's why she don't have it on the actual clearance. They just don't appear, you need to declare other names used. I will try to post a copy of my clearance via message separately. Or have your wife message me so that I can assist her.

She did wrte her maiden name on the AKA/other names used on her online application form. Went to the NBI office and specifically asked about the AKA. According to her, the person behind the counter said that it will not show on her NBi clearance. That person also was the one who changed her info about the family name etc.

I have the same information on my NBI Clearance. I got confused with people saying that I should put my married name on the information (most probably, I'm the one who misunderstood). I tried to and edited my online profile this morning and instead of my profile stating my current name "SBD" it stated "SDD" which is wrong. So I figured putting my father's name on the last name and my mother's maiden name on the middle name is correct. Husband's surname is also indicated.

With regards to the AKA, I put 2 versions of my married name on it, "SD" and "SBD" and went on with the process of printing it and going to the nearest NBI satellite. At the encoding, I was asked if I really had AKAs and told the lady that the names I put on the AKA are my current married name.

The lady told me that I have no AKAs and that AKAs are only for those who have corrections in their names or had other names used and that being married was not one of the reasons for AKA.

As far as I know, from what I've read so far, most of those that needed AKAs are those who were:

1. Married before and are now annulled

2. Those who have different versions of their names on official records (Maria Christina, Ma. Christina, Maria, Christina)

I have only used one version of my name since birth in all of my documents up until I got married. I have only been married once and still together.

Those are the two names that I have used.

I now have three NBI Clearances that bear the same information:

Last Name: Father's Surname

Middle Name: Mother's Maiden Name

First Name

Husband's Name

Meaning, I have tried to follow all the other information given to me from this forum and NBI has been consistent that the above information is correct and no AKA.

AKA is confusing and it is scary to think that visa issuance may be delayed just for the lack of it.

Yeah, I'm still confused as well because some people are saying one thing and others are saying another. But I've found no way around it since NBI, of course, would follow their format which is Last name (father's surname), Middle name (mother's maiden name), First name, husband's surname. Even if I put the format that the others are saying like putting the married name and AKAs, NBI would correct it when they encode it (which resulted to me having 3 identical NBI clearances with no AKA).

I cannot go there and tell them that they should follow the format that I've read online since that would be undermining their job/authority (it is, after all, their job and I'm hoping that, by now, they would know what and what is not needed in the forms). Not unless USEM instructs me so (which I pray would never happen), then I can go there and tell them that that's what is needed according to USEM. But right now, I don't know if the CO would reject the NBI Clearance that I have because I've also read stories where they didn't have any problems with NBI Clearances not having AKAs.

This is making me anxious. My interview will be on Nov. 13. I'll give an update on how everything turns out after the interview.
