Hello people,

Please help me out, I am trying to get papers from my HR department that shows my spouse as life insurance beneficiary and emergency contact, but they are acting like they dont know what im talking abt. Please is there a special word for this type of document? Its been 2weeks i made a request and each time i call they act confused. Please anyone who has requested and gotten this document should help me with how to explain to them, imagine a fortune 500 comp.

Isn't there an electronic system? I would think a fortune 500 company would have enterprise solutions and regardless no way HR would be confused by this. There should be some sort of benefits summary. I assume you specifically mean the life insurance through your employer and not supplemental term/life insurance.

Who does your payroll? internal? ADP?

Have you contacted the provider who actually issues the life insurance policy?

Thank you guyz;

We have a web portal, but when i print it off the portal, it doesn't look authentic, just looks ordinary like what anyone could doktor and i do not want uscis to kick back. I Had thought theres a particular document they could give me from the office that would day that,? Pls advise.

Thats probably what HR is confused by. You didnt mention it was available just not pretty. There is no std special magic doc that you are looking for. I would ask for a benefits summary sometimes you'd get like an annual mailing or something. Otherwise try the provider themselves.

Just print what you have from the web. That shouldn't be your only proof of marriage anyway - it is just one part of a larger package (lease documents/deed, joint accounts, pictures, etc.)

Thank you so much people,

My fear is i received an RFE.The IO called me back 2hrs after the interview and asked to provide her with more evidence of commingling and that In my new job she wants to see spouse as beneficiary etc. this is where the problem is. The RFE letter i got yesterday points directly to me.lol. A part says " Mr unchained, now that you are employed pls provide more documents of commingling of assets with spouse, "put more activity of your joint acct, like paying bills ,rents etc" so I want to follow her instruction to the fullest because she wasn't really smiling at interview but u could tell she has a good heart. But thanks guyz. I only started working 3weeks ago, mayb HR would be looking at me like some crazy person.lol, asking for beneficiary documents after just 3weeks. I wish i could make them understand - but i would try to get intouch with the providers directly and see what the can give me. *sorry for typos*

you should ask your co to let you add spouse as a beneficiary to the 401k. You can do this online if you are working for Fortune 500 co as these things are outsourced to 3rd party vendors. Take a print of medical benefits enrolment etc online. It need not be a actual co letterhead. but online copy is fine.

  13. 01/03/2016- EAD/AP Card Approved.
  14. 01/05/2016- Received EAD/AP Card in Post
  15. 1/12/2016- Submitting Birth certificate
