Hi Everyone,

So I need to have a vaccination record for my medical and sadly I don't have the forms from when I was a kid. It was lost years ago in one of the moves we made.

I found this list online but the Dr I saw this morning said that some of these can't be tested for (Polio, Influenza type B) thru blood work and I would need my Immunization Record as a child for that info.

• Mumps

• Measles

• Rubella

• polio

• tetanus and diphtheria toxoids

• pertussis

• influenzae type b (Hib)

• hepatitis B

• varicella

• pneumococcal and

• influenza

Today they did the following Vaccinations: Diphtheria, Tetanus, Pertussis, & Seasonal Influenza

Have to go back next week for other Vaccinations, but I am worried that if I can't get my child records because according to the Public Health office I called they only keep records 10 years after graduation and I am 25 years past graduation haha.

Will this be a major issue for me?

For the K1, you only need MMR (measles, mumps, rubella), Flu shot (if you're entering states nov-march), and TDAP (tetanus, diptheria, and pertussis). Since you have gotten the flu shot and TDAP, I believe you can go to your doctor and have them test for your MMR immunity, and go from there.

It looks like you're looking ahead to the required vaccines for adjustment of status. Sorry, not sure on that! Hopefully someone in a similar situation can help.

Thanks everyone. So the Polio and childhood vaccination stuff isn't important... As long as I have everything up to date and have my blood tested and get what I need plus have records of it all when I go to the medical I should be fine.

Great help... Thanks
