Hey guys,

A bit confused here after reading some of the posts. I'm trying to figure out with my DS-3025 will be sufficient for the AOS, or if I'll need to use the I-693 and see a civil surgeon. Here's what it says:

DT, DTP, DTaP: no dates, A

Td: 1 DATE, K1 listed under "vaccine given by panel site"

Tdap: no dates, A

OPV: no dates, A

IPV: no dates, A

MMR: 1 DATE, K1 listed under "vaccine given by panel site"

Measles: nothing

Mumps: nothing

Rubella: nothing

RotaTeq (RV5): no dates, A

Rotarix (RV1): no dates, A

Hib: no dates, A

Hepatitis A: 1 Date, A

Hepatits B: no dates, A

MVC4: no dates, A

Other MVC: no dates, A

Varicella vaccine: no dates, D

Varicella history: checked, no dates no code

PCV 7: A

PCV10: A

PCV 13: A

PPSV23: A

Influenza: F

Nothing is checked in part 2.

Can anyone please let me know if this checks out, and what vaccines we need to get?
