Hi guys I just want to share my medical experience in St. Luke's Extension Clinic located along Bocobo Street.

So here it goes...

Day 1 (October 21, 2015)

I woke up an hour before my alarm which was supposed to be 3:30am.So I was very sleepy and nervous at the same time.

I brought:

1) Online registration (can be found on the SLEC website)

2) Passport PLUS the photocopy of the page where it shows your information.

3) NVC Appointment letter or the interview letter, whatever you can call it

4) 2x2 photos (3 pcs or 4?) and put your name on the back of every photo

5) Immunization record (depends on you. when you don't have it anymore, it's okay)

6) For minors like me (though I'm with my guardian), you need an authorization letter

We arrived there at 5:30 and there was a line already. My number was 51 but when I heard the number calling inside, it was already on 20s so I thought that "oh that was fast". But when my guardian and I reached the counter, I found out thatthe documents I brought were incomplete. I needed an authorization letter from my mom saying that she's allowing my godmother to go with me to the clinic. So we had to go outside to find an internet cafe, good thing that there's already an internet cafe open just in front and my mom's online so she emailed it to me as soon as possible then I printed three copies. When we went back, we headed straight right to the counter because the clerk said that I could go straight to him when I'm complete. Before I reached him, he called someone to check and arrange my files so it would be fast when I reach him.

When everything went okay, he told me to go the 5th floor to pay and we paid 11, 515 (I'm 17 yo).

Here's the list of what I've done:(not in order)

1) Xray

2) Blood getting

3) Vital signs

4) Height and weight plus the eye test

5) Immunization interview

6) Physical exam (sorry but everyone needs to strip. YUP I WANTED TO CRY)

After all that, they said that we just need to go back tomorrow then we did a while ago.

Day 2 (October 22,2015)

The lady said that I need to go back at 2pm yesterday but we arrived there at around 8am, thinking that I need to get my vaccine first then we'll just wait til 2pm... But but but, (wait! read my experience firstcthen you'll see hahaha)

Before I get inside, I gave my receipt (DON'T MISPLACE THIS, THIS IS VERY IMPORTANT) to the guard and it's stamping time, I was very nervous because they said that if you get an IMMUNIZATION stamp, then you passed. If you get SPUTUM, then you need to cancel your interview I forgot why. There are anpther that I've read online, but I'm not surewith them anymore. So I went straight to the 2nd floor. I tried to be calm as much as possible hahaha still it was nerve-wracking and when my name was called, I went to the immunization room to get my shots. I've got no immunization record because it went missing a long time ago that's why I needed to take 6 shots (If I'm not mistaked, this depends on your age). Thank God I did not faint!!! Hahaha exaggerated, yes, but who cared. That was scary! lol. Then the nurse told us to eat first because he heard my godmother and I talking about not having our breakfast yet.

When we went back, we just stayed on the first floor. I forgot to tell you guys that the nurse said that we just need to wait for my name to be called on the counter (1st floor) when we get back.

I was called two or three times to the counter just to sign some papers and the last time I was called, she just confirmed something (like my basic infos) and when everything were just eight, she gave me the envelope and the CD (CD I think for the xray result), then she told me to bring the following items to my interview and wait for that time to come.

PS: DON'T DARE OPEN THEM HAHAHAHAHA

3/03/16 - Received text and email notification

3/04/16 - Check cashed

3/07/16 - I-797 hardcopy received

3/19/16 - Biometric scheduled received by mail

4/06/16 - I-485 online status changed to Ready to be Scheduled for Interview

4/15/16 - I-485 online status changed to INTERVIEW SCHEDULED

4/17/16 - Interview Notice hardcopy received

4/28/16 - AP & EAD approval date

5/02/16 - AP approval letter received

5/05/16 - EAD card received

5/17/16 - AOS interview

5/23/16 - Online status changed to CASE APPROVE dated 5/18/16

5/23/16 - Received AOS approval letter in the mail

5/23/16 - Online status changed to CARD WAS MAILED TO ME

5/24/16 - Card was picked-up by USPS (online status)

5/26/16 - Green Card on Hand!!! :goofy:
