I am so confused right now. ?? I was replying to this thread topic and going to quote a reply and then it was gone. Hehe. I'm so sorry if I hijacked/duplicated another thread.

I know the AKA is for the corrections or other names used by an individual, and it needs to appear on the NBI clearance.

So my wife's NBI Clearance is correct then?

It's only correct if you have followed what is contained in the statement I made earlier. Here it is again. The red highlighted text explains it completely. I hope this helps.

NBI CLEARANCE (USEM requires Green Form). Applicants aged 16 years and older must have a valid Record Clearance (purpose: for travel abroad) from the National Bureau of Investigation (NBI). Clearances should be in the applicant’s current name, birth certificate name, maiden name and any aliases or nicknames used, including different spellings you have used of those names. An official letter of explanation from the NBI is required for any notation of "No criminal record" or "No pending case". The NBI clearance is valid for one (1) year from the date it was issued but the USEM Consulate requires it to be no older than 6 months at the time of the interview. Return and get a new clearance if it is older than 6 months. The NBI website is http://www.nbi.gov.ph.

Hi Greenbaum. Thank you for that information. I know what it says since I have read it a couple of times. The only reason Im asking , and why I attached a picture of the said Clearance is Because I wanted to know if that's the way its supposed to look like. I don't know how the clearance should look now, the NBI clearance the last I saw one was not like it.

If it is the same NBI everyone else(from the Philippines) sent and have at the time of the interview.

Thank you so much for your help.

Did you receive that NBI from the NBI office? Then that is how it should look. Changing form design is common.

There are two versions of an NBI, yellow for local, and green for travel abroad, etc.

Follow the embassy instructions letter. (BTW can't get letter until the interview ;) but you don't need it)

POLICE REPORT (NBI Clearance). Applicants aged 16 years and older must have a valid Record Clearance (for travel abroad purposes) from the National Bureau of Investigation (NBI). An official letter of explanation from the NBI is required for any notation of “No criminal record” or "No pending case”. For immigration purposes, an NBI clearance is valid for one (1) year after its issuance date. NBI’s website is http://www.nbi.gov.ph.

If your form says something like NO RECORD ON FILE or NO DEROGATORY RECORD then you have nothing to worry about. That will be good for the interview. Good luck.
