For the form I-693, does it really need to be transcribed by a surgeon? I didn't have any problem with medical whatsoever when I had it in the Philippines. Also, I got all the shots that I needed. So I'm just wondering if my medical needs to be transcribed by a surgeon. Your insights are highly appreciated! :)

If you have the DS-3025 from St Lukes, just send in a copy of that with your AOS application.

hi gisselle pls. update me if you dont encounter any problems by just sending the ds-3025. so just to clarify you didnt send any i-693? just the ds 3025 right?

We only sent in a copy of the DS-3025. We were approved no problem.
