I'm back again with yet another question. Hopefully its the last one :)

for part 6 question 2 do i enter Pretax or after tax. another words is it net or gross ?

for part 11 questions 1a and 1b do i enter my name ? even though no notes were entered on the page?

You enter your gross income. If your salary is X, you enter X. This is how much gross income you'll make from today through the next year. Second question is no. Not only must you read the instructions but you must endeavor to understand the context of each question.

A previous response wrote something about line 22 of your 1040 form. If it's on a 1040 form, unless you're self employed, it does not apply to your current income question. The numbers from line 22 of three tax returns are entered elsewhere and refer to past income, not current income.

If your taxes are a 1040 form put line 22 and yes put your name there and put n/a in the notes .

I just recently received the 1040 form along with the w2 and 1099R so things are making a lot more sense now. I appreciate all your input
