First I would like to thank all of those VJ members who have been helpful in me and my wife's journey.

I checked the status on the CEAC website this morning and it shows "Issued ... Your visa has been printed. Depending on local procedures at the location where you were interviewed, your visa will be mailed or available for pickup soon. If there are further questions, or if we need updated contact information, you will be contacted.

For more information, please visit TRAVEL.STATE.GOV."
Has the visa packet already left the consulate? And if so is there a way to track it until it reaches its destination?

Yes, you can track your passport on us travel docs website. Just Google travel docs track passport italy. What info you see is very limited. For me it said "the document left the embassy and is being processed for delivery" and then a day later "ready for pick up".

Yes, I would definitely call the embassy to inquire about your visa as 4 weeks is too long.
