Hi,

I got interview letter by email. and my pd is 10nov2015. Now next step is Medical and I confused about Vaccination Record or history. I dont have any vaccination history. I live in Ontario Canada and here they need my vaccination history and they have checklist which contain following shots:

1) Td/TdAP

(must have had in last 10 yrs)

2) MMR

3) VARICELLA

(If had Chicken Pox before then not needed)

4) FLU SHOT

So do I have to get these shot ? The Panel physicians who do medical dont give these shot

Do you not have the shots or do you not have the records?

Most public schools keep records of your vaccines, I called my school board and they were able to get the record faxed over to me the same day. Otherwise you can have a titre test to find out which immunities you have.

April 29, 2013 - NOA1 for petition received

September 10, 2013 Interview - decision could not be made.

April 15, 2014 APPROVED. Wait for oath ceremony

Waited...

September 29, 2015 - sent letter to senator.

October 16, 2015 - US Citizen

Timeline

I got most of mine from my schools. I hadn't been in school for 20 years and was surprised that they still had them on file. I was missing something and simply went to the doctor and got this shot - flu I think.

5.26.23: Wedding

8.21.24: NVC
