I've got Blue Cross Blue Shield through my husband's employer, they have been covering everything preexisting since I've had it (I could have delivered a baby one day after getting married, or started cancer treatment). Affordable Care act pushed these rules further. Google it if you need details, on my phone now.

I think the exception is visitors health insurance.
