How does one know the filing location before anything is even sent?

Does the affadavit of support have to be included with the I129f?

1. You don't. You send the application to the Dallas Lockbox, as clearly stated in the instructions. They will forward it to the right processing center. As of March this year all cases have been routed to California to ease backlog in the Texas Service Center (which is not the lockbox!). How long that'll last, no one knows.

2. No, you take these to the interview or send them to the embassy, depending on the country-specific process.

the two passport "style" photos?. I have two small photos of my fiance that are small like passport photos and they are two months old. Will they accept these?

The photos must be actually passport photos, you will need one for yourself and one for your fiance

As for the LoI you use the address of the lockbox

No address is needed on the letter of intent that you include with the I-129 package.

You must have 2 wet signatures, meaning either one letter with both of your original signatures or one letter from each of you with

each of your original signature, no scanned copies.

Just put "To whom it concerns:" and be done with it.
